Three Brains, One Healing Conversation: Understanding the Nervous System and MS

After twenty-five years in clinical practice—and about a decade of watching this particular area of science evolve—I’ve come to think about the nervous system differently.
We tend to imagine that we have one brain: the brain inside our skull, directing everything from above.
But the reality is much more interesting.
Your brain, gut, and heart are all involved in an extraordinary network of communication. They constantly exchange information through nerves, hormones, immune signals, and the autonomic nervous system.
I sometimes describe this as “three brains, one conversation.”
It isn’t meant to suggest that your gut or heart is literally a second or third brain in the same sense as your brain and spinal cord. Rather, each contains extensive neural networks and participates in regulating how your body responds to the world around you.
And once you begin to understand that conversation, it can change how you think about your nervous system—and about healing, particularly when you're living with a chronic condition like multiple sclerosis (MS).
Because healing isn't simply something that happens to you.
Your body is participating in the conversation, too.
Your Nervous System Isn't Just in Your Head
When most of us hear the words “nervous system,” we picture the brain and spinal cord: the central command center that receives information, processes it, and coordinates a response.
That picture isn't wrong.
It's just incomplete.
Your nervous system extends throughout your body, and some of its most important communication happens between your brain and the organs inside you.
Your Brain and Spinal Cord
Your central nervous system is continuously gathering information about your internal and external environment.
Is there danger?
Are you safe?
Do you need to become more alert?
Do you need to conserve energy?
Much of this assessment happens automatically, before you consciously recognize what is happening.
This is one reason simple grounding practices can sometimes be helpful. Bringing your attention to what you can see, hear, and feel in the present moment can help shift your attention away from perceived threat and toward what is actually happening around you.
It's a small practice, but it can be a meaningful way of reconnecting with your body.
Your Gut: The Enteric Nervous System
You've probably heard the gut called the “second brain.”
There is a reason for that.
The digestive tract contains an extensive network of neurons known as the enteric nervous system. It can coordinate many aspects of digestion independently while also communicating constantly with the brain.
And the communication goes both ways.
Your gut is influenced by the brain, while signals from the gut travel back toward the brain through neural, hormonal, metabolic, and immune pathways.
This is part of what researchers often call the gut-brain axis.
Your gut microbiome is part of this larger picture, too. The trillions of microorganisms living in the digestive tract interact with the immune and metabolic systems and may influence signaling throughout the body.
This doesn't mean every sensation in your gut is a message you should automatically trust.
It means your gut is not disconnected from your brain.
It is part of the conversation.
Your Heart: More Than a Pump
The heart is another fascinating participant.
The heart contains its own intrinsic nervous system and continuously sends information to the brain about its rhythm, pressure, and physiological state.
At the same time, the brain influences the heart through the autonomic nervous system.
Think about what happens when you suddenly become frightened.
Your heart may begin beating faster.
Your breathing changes.
Your muscles prepare for action.
Your digestive system may slow down.
And all of this can happen before you've had time to consciously think, I'm scared.
That's the nervous system doing exactly what it was designed to do: rapidly coordinating information throughout the body.
The Vagus Nerve: One of the Body's Major Communication Pathways
If the brain, gut, and heart are constantly communicating, how does some of that communication happen?
One important pathway is the vagus nerve.
The vagus nerve is a major component of the parasympathetic nervous system and connects the brain with several organs in the body, including the heart and digestive tract.
Importantly, much of the information carried by the vagus nerve travels from the body toward the brain.
That's an important shift in perspective.
We often imagine the brain sending instructions outward.
But the brain is also receiving an enormous amount of information from the body.
Your body is not simply waiting for orders.
It is continuously reporting back.
It's a conversation. Three brains, one conversation, and it's been running since before you could speak.
You've Been Listening to This Conversation Your Whole Life
Think about the last time you had a “gut feeling” about something.
Or the moment your heart seemed to drop after hearing unexpected news.
Or the way your breathing changed when you walked into a stressful situation.
These experiences aren't proof that every intuition is physiologically accurate.
But they do reflect something real: your brain is constantly receiving information from your body.
We don't always consciously notice that information.
Sometimes the body responds before our conscious mind has had time to put the experience into words.
Imagine walking alone through a dark parking lot and suddenly hearing a noise behind you.
Before you've consciously identified the sound, your attention shifts.
Your heart may accelerate.
Your breathing may change.
Your muscles may tense.
Your digestive system may temporarily become less of a priority.
Your nervous system has already begun responding.
Only afterward does your conscious brain say, What was that?
That isn't mystical.
It's physiology.
Why the Nervous System Matters When You're Living With MS
This becomes particularly interesting when we talk about multiple sclerosis and the nervous system.
MS is a complex neurological disease involving the immune system and central nervous system. Medical treatment remains an essential part of managing MS, and nervous system regulation should never be presented as a replacement for evidence-based MS care.
But there is another piece of the picture worth understanding.
Your nervous system doesn't exist separately from the rest of your physiology.
Stress, sleep, movement, breathing, emotions, social connection, and your perception of safety can all influence how your body functions.
That doesn't mean stress causes MS.
It doesn't mean nervous system regulation can cure MS.
And it certainly doesn't mean someone with MS is responsible for their symptoms because they haven't “regulated” their nervous system enough.
Those are dangerous oversimplifications.
What it does mean is that your nervous system is part of your overall health environment.
And that gives you something important:
Agency.
Not complete control.
Not a promise of a cure.
But agency.
Stress and Nervous System Regulation
When stress becomes chronic, the body can spend more time in a state of heightened physiological arousal.
Learning ways to regulate that response may support overall well-being, sleep, emotional resilience, and the body's ability to move between states of activation and recovery.
For someone living with MS, these supportive practices can become part of a broader approach to health alongside appropriate medical care.
That's an important distinction:
Alongside—not instead of.
Your Breath: A Simple Way to Work With Your Nervous System
Of all the practices I could suggest, I keep coming back to something wonderfully simple.
Your breath.
You don't need a device.
You don't need a supplement.
You don't need an expensive appointment.
You've been carrying this tool with you your entire life.
Slow, controlled breathing can influence autonomic nervous system activity, and breathing patterns are closely connected with heart rate and other physiological processes.
One simple practice is to make the exhale slightly longer than the inhale.
For example:
Inhale for 4 seconds.
Exhale for 6 seconds.
You don't need to force the breath or make it unusually deep.
The goal is simply to slow down and create a little more space between one breath and the next.
And if counting makes you anxious or uncomfortable, forget the numbers.
Just breathe gently and comfortably.
What Is HRV and Why Does It Matter?
This brings us to heart rate variability, or HRV. You may have seen HRV on a smartwatch or fitness tracker.
HRV is not the same thing as your heart rate.
Instead, it describes the variation in timing between individual heartbeats.
That variation is normal—and meaningful.
A healthy nervous system isn't supposed to operate like a metronome. It needs flexibility.
Your heart rate naturally changes in response to breathing, movement, emotion, posture, sleep, stress, and many other factors.
Higher HRV isn't automatically “better,” and a single HRV reading shouldn't be treated as a diagnosis.
But HRV can provide one window into how the autonomic nervous system is responding and adapting.
And breathing is one of the simplest ways to influence this heart-brain interaction in real time.
The Bigger Picture: Healing as Participation
This is ultimately what I hope you take from the idea of three brains and one conversation.
Your brain, gut, heart, and the rest of your nervous system are not operating in isolation.
They are communicating constantly.
The question isn't whether that conversation is happening.
It already is.
The question is whether you can begin to notice it.
Can you notice what happens to your breathing when you're stressed?
Can you recognize when your body is asking for rest?
Can you notice how your gut responds to certain situations?
Can you recognize the difference between being genuinely safe and simply being accustomed to feeling threatened?
These observations aren't a substitute for medical evaluation.
They're a way of becoming more connected to the body you're living in.
And that's an important part of what I mean when I talk about living beyond the diagnosis.
It doesn't mean ignoring MS.
It doesn't mean pretending your diagnosis doesn't exist.
And it certainly doesn't mean trying to heal yourself instead of working with your healthcare team.
It means recognizing that you are more than your diagnosis—and that you can participate actively in your health journey.
Your medical treatment matters.
Your relationships matter.
Your sleep matters.
Your movement matters.
Your stress response matters.
Your breath matters.
And so does the way you listen to your own body.
